The DJL0 patch here fixes the delay pools starvation issue.
I believe this will be included in Squid 2.3.DEVEL3 with some
changes, the most significant being the addition of a missing
FD_ZERO of slowfds in the loop in comm_poll(), which is a bad
but non-critical problem so I haven't produced a new patch.

The DJL1 patch is the (untested and at least partly broken)
byte-charging code.
